Generally the dental insurance policy will state if it covers orthodontic work and how much it covers. Usually for a PPO type plan, the amount (say $1000 of coverage) is up to 50%, meaning when you have charges of $2000, it will cover 50%, up to the limit of $1000. If it's a DMO/HMO plan it will state if orthodontic work is covered and how much is covered in the policy.





For a PPO policy, the payments are made at 50% as they happen, meaning the installation is covered at 50% (which usually almost eats up a $1000 policy anyway) and then other appointments (not maintenance) are covered at 50% as they happen. Then whatever amount is left to pay for the monthly maintenance appts is split up over the length of the treatment (say 24 months) and each payment amount is usually less than 50% of your monthly bill. Confusing, huh? = )





So if you had $240 left for monthly maintenance, they'd pay you at $240 /24 = $10 per month, even though the cost for the appts may be $200. At least that's how the insurances we've had have worked.
