Generally the dental insurance policy will state if it covers orthodontic work and how much it covers. Usually for a PPO type plan, the amount (say $1000 of coverage) is up to 50%, meaning when you have charges of $2000, it will cover 50%, up to the limit of $1000. Then whatever amount is left to pay for the monthly maintenance appts is split up over the length of the treatment (say 24 months) and each payment amount is usually less than 50% of your monthly bill. So if you had $240 left for monthly maintenance, they'd pay you at $240 /24 = $10 per month, even though the cost for the appts may be $200. At least that's how the insurances we've had have worked.
